ABSTRACT

ABSTRACT The Sphenoid Sinus is considered the paranasal sinus with more variation to the degree of pneumatization. Thus, this work aimed report to the first case on the interference of this anatomical variation for orthognathic surgery. A 18- year-old woman, with isolated cleft palate repaired, was submitted to orthognathic surgery to correct the maxillomandibular skeletal discrepancy. The cone beam computed tomography (CBCT) exam performed for preoperative planning showed a great extension for the adjacent structures, with proximity to the posterior wall of the maxillary sinus and pterygomaxillary fissure bilaterally. The postoperative CBCT image evidenced the compromise of the sinus floor due to the factors related to the transoperative period. This case, along with literature review, highlighted the importance of identifying sphenoid sinus variation in CBCT exams prior to orthognathic surgery, in order to avoid serious complications for the patient, such as sinus infections; hemorrhages; mucocele formation or intracranial involvement.


RESUMO O Seio Esfenoidal é considerado o seio paranasal com maior variação quanto ao grau de pneumatização. Assim, este trabalho objetivou relatar o primeiro caso sobre a interferência dessa variação anatômica para a cirurgia ortognática. Mulher,18 anos, com fissura de palato isolada reparada, foi submetida à cirurgia ortognática para correção da discrepância esquelética maxilomandibular. O exame de tomografia computadorizada de feixe cônico (TCFC), realizado para planejamento pré-operatório, mostrou grande extensão para as estruturas adjacentes, e com proximidade da parede posterior do seio maxilar e fissura pterigomaxilar bilateralmente. A imagem de TCFC pós-operatória evidenciou o comprometimento do assoalho do seio devido aos fatores relacionados ao transoperatório. Este caso, juntamente com a revisão da literatura, destacou a importância de identificar a variação do seio esfenoidal nos exames de TCFC prévios à cirurgia ortognática, a fim de evitar complicações graves para o paciente, tais como sinusite, hemorragias, formação de mucoceles ou, até mesmo, envolvimento intracraniano.

Abstract During times of increasingly recognized importance of interprofessional practices, professionals in Medicine, Dentistry, and Speech Pathology areas cooperate to optimize treatment of velopharyngeal dysfunction (VPD), after primary palatoplasty for correction of cleft palate. Objective Our study aims to compare velar length, velar thickness, and depth of the nasopharynx of patients with unilateral cleft lip and palate (UCLP) with the presence, or absence, of hypernasality and nasal air emission; and to verify if the depth:length ratio, between nasopharynx and velum, would be predictive of consistent hypernasality and nasal air emission (speech signs of VPD). Methodology Cephalometric radiographs and outcome of speech assessment were obtained from 429 individuals, between 6 and 9 years of age, with repaired unilateral cleft lip and palate. Velar length, velar thickness, depth of the nasopharynx, depth:length ratio, scores of hypernasality, and scores of nasal air emission were studied and compared; grouping the radiographs according to presence or absence of hypernasality and nasal air emission. Results For the group with speech signs of velopharyngeal dysfunction (those with consistent hypernasality and nasal air emission), the velums were shorter and thinner; the nasopharynx was deeper and the depth:length ratio was larger than the group without hypernasality and nasal air emission. Velar length was significantly shorter in individuals with consistent hypernasality and nasal air emission (p<0.001) and with history of palatal fistula (p=0.032). Depth of nasopharynx was significantly greater in individuals with consistent hypernasality and nasal air emission (p<0.001). Depthlength ratio was significantly larger in individuals with consistent hypernasality and nasal air emission (p<0.001). A depth:length ratio larger than 0.93 was always associated with speech signs of VPD. Conclusion Estimated with cephalometric radiographs, a depth:length ratio greater than 0.93, between the nasopharyngeal space and the velum, was 100% accurate in predicting hypernasality and nasal air emission after primary repair of unilateral cleft lip and palate.

ABSTRACT

This paper aims to report a case of radicular displacement to the submandibular space and to review the literature seeking reports of dental displacements / fragments published in the period from 2007 to 2017 in PubMed evidencing risk factors, prevention and forms of treatment. In this article a case of displacement of dental fragments to the submandibular space with immediate removal is reported. The third molar or its roots displacement into facial spaces is a rare situation. The preoperative surgical planning is fundamental to avoid this type of complication. The association of (1) good surgical planning, (2) surgical technique utilization and adequate instruments, and (3) the surgeon experience are determining factors to avoid the occurrence of this complication. A extração de terceiros molares é o procedimento cirúrgico mais frequente nos consultórios odontológicos. Como qualquer cirurgia, existe a possibilidade de complicações trans e pós-operatórias. O deslocamento desses dentes ou fragmento dentário para espaços faciais é raro, com frequência extremamente baixa. Este trabalho tem como objetivo relatar um caso de deslocamento radicular para o espaço submandibular e revisar a literatura buscando relatos de deslocamentos dentários/fragmentos publicados no período de 2007 a 2017 no Pubmed evidenciando fatores de risco, prevenção e formas de tratamento. Nesse artigo nós relatamos um caso de deslocamento de um fragmento radicular para o espaço submandibular, sendo este removido imediatamente. O deslocamento do terceiro molar ou de fragmentos radiculares para os espaços faciais é raro. A avaliação pré-operatória é fundamental para evitar esse tipo de complicação. A associação entre (1) planejamento cirúrgico correto, (2) utilização de técnica e materiais corretos e (3) experiência do profissional, são fatores determinantes para evitar esse tipo de complicação. ABSTRACT: Florid osseous dysplasia (FOD) is an extensive form of osseous dysplasia where normal trabecular bone is substituted by fibrous connective tissue and amorphous mineralized tissue. Usually, the lesions are mainly asymptomatic and the patients should be followed with clinical and imaging examination, requiring no intervention. Nevertheless, due to the poor vascularization of the lesion and to local trauma, secondary infections and osteomyelitis may occur. Patients may present with pain, mucosal ulceration, lesion exposure in the oral cavity, fistula and swelling. In such cases, the correct diagnosis and management of the lesion is decisive to reestablish patient's health and quality of life. The aim of this article is to present a case of complicating secondary chronic osteomyelitis treated successfully with conservative intervention. A 68-year-old black female patient reported a "swelling of the gums" that was present for eleven years, with episodes of periodic pain and swelling. On physical examination, a papule with suppuration in the alveolar mucosa in the right side of the mandible was observed. Panoramic radiography and CBCT showed a mixed lesion surrounded by sclerotic bone. The patient was diagnosed with chronic osteomyelitis in association with FOD and treated with antibiotic therapy together with surgical curettage. The incidence, etiophatology, differential diagnosis, treatment and prevention of secondary osteomyelitis associated with FOD are discussed in the light of literature. This information might assist the dentists while choosing the best treatment options for similar cases.


RESUMEN: La displasia ósea florida (DOF) es una forma de displasia ósea donde el hueso trabecular normal es sustituido por tejido conectivo fibroso y tejido mineralizado amorfo. En general, las lesiones son de origen asintomático y los pacientes deben ser seguidos con el examen clínico. Sin embargo, debido a la pobre vascularización de la lesión y al trauma local, pueden producirse lesiones secundarias y osteomielitis. Los pacientes pueden presentarse con dolor, ulceración mucosa, lesión de exposición en la cavidad oral, fístula y edema. En estos casos, el correcto diagnóstico y tratamiento de la lesión es decisivo para reestabilizar la salud y la calidad de la vida. El objetivo de este artículo consistió en presentar un caso de complicación secundaria crónica de osteomielitis tratados con tratamiento conservador. El paciente, negro, de 68 años de edad, consulta por "hinchazón de las encías" que se presentó durante once años, con episodios de dolor. En el examen intraoral, se observó una pápula con supuración en la mucosa alveolar en el lado derecho de la mandíbula. La radiografía panorámica y CBCT mostraron una lesión mixta rodeada de hueso esclerótico. El paciente fue diagnosticado con osteomielitis crónica en asociación con DOF y fue tratado con tratamiento antibiótico junto con curetaje quirúrgico. La incidencia, la etiopatología, el diagnóstico diferencial, el tratamiento y la prevención de la osteomielitis secundaria asociada con DOF se discuten a la luz de la literatura. Esta información puede ayudar a los dentistas a elegir las mejores opciones de tratamiento para casos similares.

ABSTRACT

Objetivo: avaliar a prevalência do defeito osteoporótico focal nos maxilares (DOFMO) através de imagens radiográficas. Material e métodos: neste estudo transversal e retrospectivo, tomografias computadorizadas de feixe cônico (TCFC) – voxel = 0,2 mm, 120 KVp, 36,12 mA – foram selecionadas a partir de um banco de dados e analisadas em um programa de computador específico. O DOFMO foi definido como uma imagem hipodensa no trabeculado medular, circunscrita, sem limites muito definidos e sem envolvimento cortical, ausência de expansão, sem aspectos de lesão cística e nenhum envolvimento odontogênico, delimitado por linhas hiperdensas finas e contínuas separadas do trabeculado ósseo vizinho. Dois examinadores calibrados foram constituídos, e as discordâncias foram resolvidas por um terceiro avaliador. Resultados: o número total de TCFCs investigadas foi 525 (maxila = 258, mandíbula = 267), sendo 195 homens e 330 mulheres, com média de idade de 50,7 anos. Foram observados 91 DOFMOs (17,3%), sendo 32 (61,5%) no sexo feminino e 21 (38,4%) no sexo masculino. Em 44 casos (83,1%), observou-se o defeito na mandíbula; em nove casos (6,9%), observou-se o defeito na maxila. Em 38 casos (71,7%), o DOFMO se deu bilateralmente; em 15 casos (28,3%), foi observada distribuição unilateral, sendo que 12 foram encontrados na mandíbula e três na maxila. Conclusão: através deste estudo, pôde-se concluir que o DOFMO não se apresentou incomum e deve ser considerado durante o planejamento cirúrgico de instalação de implantes dentários.


Objective: to evaluate the prevalence of osteoporotic focal defect in the jaw (OFDJ) through radiographic images. Material and methods: in the cross-sectional retrospective study, CBCTs (voxel = 0.2 mm, 120 KVp, 36.12 mA) were selected from a databank and analyzed with a specifi c computer program. OFDJ was defined as circumscribed, hypodense image at the marrow area without defined limits without cortical involvement, no expansion, no aspects of cystic lesion and no odontogenic involvement, bounded by thin and continuous hyperdense lines separated from the adjacent trabecular bone. Two calibrated examiners made the diagnosis; disagreements were arbitrated by a third reviewer. Results: 525 CBCTs were investigated (maxilla = 258; mandible = 267), being 195 men and 330 women (mean age of 50.7 years). 91 OFDJ cases (17.3%) were seen, being 32 (61.5%) female and 21 (38.4%) and male. In 44 cases (83.1%) there was a mandibular defect, whereas 9 cases (6.9%) were seen in the maxillary arch. Bilateral distributions were seen for 38 cases (71.7%), while the unilateral pattern was observed only for 15 cases (28.3%), being these 12 in the mandible and 3 in the maxilla. Conclusion: OFDJ is not uncommon and must be considered during the planning for dental implant placement.

ABSTRACT

Treatment of patients with cleft lip and palate is completed with fixed prostheses, removable, total, implants and aims to restore aesthetics, phonetics and function and should be guided by the basic principles of oral rehabilitation, such as physiology, stability, aesthetics, hygiene and the expectations of the patient. In order to obtain longevity of a prosthetic rehabilitation, the periodontal and dental tissue as well as the biomechanics of the prosthesis are to be respected. The purpose of this article is to describe the types of prosthetics treatment, which are performed at HRAC/USP for the rehabilitation of cleft area in adult patients.

ABSTRACT

This paper presents the treatment protocol of maxillofacial surgery in the rehabilitation process of cleft lip and palate patients adopted at HRAC-USP. Maxillofacial surgeons are responsible for the accomplishment of two main procedures, alveolar bone graft surgery and orthognathic surgery. The primary objective of alveolar bone graft is to provide bone tissue for the cleft site and then allow orthodontic movements for the establishment of an an adequate occlusion. When performed before the eruption of the maxillary permanent canine, it presents high rates of success. Orthognathic surgery aims at correcting maxillomandibular discrepancies, especially anteroposterior maxillary deficiencies, commonly observed in cleft lip and palate patients, for the achievement of a functional occlusion combined with a balanced face.

Introdução: a utilização de substitutos ósseos vem aumentando na Odontologia em decorrência de resultados clínicos satisfatórios e previsíveis, menor morbidade pós-operatória e preços acessíveis. O material mais utilizado ainda é o osso bovino inorgânico com propriedades osteocondutoras. Na década de 80 iniciaram as buscas por um material sintético, osteocondutor com resultados iguais ou superiores ao osso inorgânico bovino. Objetivo: comparar histológica e histometricamente a propriedade osteocondutora do osso composto (GenMix, Baumer) com o fosfato betatricálcio (GenPhos, Baumer) implantados em defeito de tamanho crítico em calvária de rato. Resultados: observou-se 32,5% de neoformação óssea no grupo do osso composto e 45,9% no grupo do fosfato betatricálcio. Conclusões: com o tempo de observação microscópica desse trabalho, é possível afirmar que os materiais estudados não são absorvíveis e que o fosfato betatricálcio é mais osteocondutor comparado ao osso composto.


Introduction: the use of bone replacements in Dentistry has increased as a result of satisfactory and predictable clinical results, lower postoperative morbidity and affordable prices. The most common material used is still the inorganic bovine bone with osteoconductive properties. In the 1980’s began the search for a synthetic material with osteoconductive results equal or superior to inorganic bovine bone. Objective: To compare histological and histometric property of osteoconductive bone compound (GenMix, Baumer) with phosphate beta- tricalcium(GenPhos, Baumer) implanted in critical size defects in rat calvaria. Results: It was observed 32.5% of new bone formation in the group of compound bone and 45.9% in the phosphate beta-tricalcium group. Conclusions: Along the time of microscopic observation of this work, one can state that the materials studied are not absorbable and that phosphate beta-tricalcium is more osteoconductive compared to the compound bone...

ABSTRACT

The aim of this article is to present the pediatric dentistry and orthodontic treatment protocol of rehabilitation of cleft lip and palate patients performed at the Hospital for Rehabilitation of Craniofacial Anomalies - University of São Paulo (HRAC-USP). Pediatric dentistry provides oral health information and should be able to follow the child with cleft lip and palate since the first months of life until establishment of the mixed dentition, craniofacial growth and dentition development. Orthodontic intervention starts in the mixed dentition, at 8-9 years of age, for preparing the maxillary arch for secondary bone graft procedure (SBGP). At this stage, rapid maxillary expansion is performed and a fixed palatal retainer is delivered before SBGP. When the permanent dentition is completed, comprehensive orthodontic treatment is initiated aiming tooth alignment and space closure. Maxillary permanent canines are commonly moved mesially in order to substitute absent maxillary lateral incisors. Patients with complete cleft lip and palate and poor midface growth will require orthognatic surgery for reaching adequate anteroposterior interarch relationship and good facial esthetics.

Cleft lip and palate is the most common among craniofacial malformations and causes several esthetic and functional implications that require rehabilitation. This paper aims to generally describe the several aspects related to this complex pathology and the treatment protocol used by the Hospital for Rehabilitation of Craniofacial Anomalies, University of São Paulo (HRAC-USP) along 40 years of experience in the treatment of individuals with cleft lip and palate.

ABSTRACT

O objetivo do presente estudo consistiu em realizar a análise cefalométrica computadorizada retrospectiva da projeção maxilar e de seus efeitos sobre os tecidos moles da região anterior da maxila de 15 pacientes portadores de deformidade dentofacial esquelética caracterizada por prognatismo mandibular (Padrão III). Seis indivíduos eram do gênero masculino e 9 do gênero feminino, com idade média de 26,06 anos (desvio-padrão de 7,34). Esses pacientes foram submetidos à cirurgia ortognática para avanço de maxila, por meio da osteotomia Le Fort I, e recuo de mandíbula, por meio da osteotomia sagital bilateral da mandíbula. As radiografias cefalométricas pré e pós-operatórias, com intervalo médio de 30,13 meses (desvio-padrão de 15,76), foram analisadas com base na comparação de medidas lineares e angulares por meio da utilização do programa Dolphin Imaging® 10.0 (Dolphin/3M - EUA) e os dados obtidos foram submetidos à análise estatística pelo teste não paramétrico de Wilcoxon. Ocorreu redução significativa da projeção nasal e aumento não significativo da base nasal e do ângulo nasolabial. Os valores do SNA confirmam o avanço da maxila e os valores do SNB o recuo de mandíbula, enquanto que o ANB e o trespasse horizontal tornaram-se positivos. Pode-se concluir que o avanço maxilar produz efeitos na projeção maxilar e nos tecidos moles sobrejacentes. Tais efeitos, na maioria dos casos, mostram uma tendência ao aumento das medidas lineares e angulares envolvidas, porém essa tendência pode variar de acordo com o montante e direção do movimento, além de características relacionadas aos indivíduos e aos procedimentos.

OBJETIVO: ciente de que a cirurgia ortognática moderna se preocupa em planejar e diagnosticar os casos clínicos utilizando medidas obtidas de grandezas do perfil tegumentar dos pacientes - com o auxílio de imagens digitais empregadas em softwares de planejamento -, no presente estudo foi proposto aferir as medidas de brasileiros leucodermas de descendência européia e compará-las com as medidas já padronizadas por Arnett, com o intuito de criar novas medidas a serem seguidas por brasileiros que utilizam o software de predição cirúrgica Dolphin Imaging 9.0. MÉTODOS: foram utilizadas radiografias cefalométricas de 31 pacientes com oclusão Classe I de Angle e harmonia facial. Todas as radiografias foram digitalizadas e inseridas no software Dolphin 9.0 e 16 pontos de tecido mole e 22 pontos do esqueleto facial foram marcados, seguindo-se exatamente as marcações da análise de Arnett e McLaughlin presentes no programa. RESULTADOS: os resultados obtidos foram avaliados estatisticamente e mostram que o perfil do brasileiro é quase totalmente diferente do perfil norte-americano, exceção feita a apenas quatro pontos para os homens e outros quatro para as mulheres. Os brasileiros apresentam uma face menos protruída, um perfil mais convexo e menor proeminência do queixo do que o grupo controle. CONCLUSÃO: esses dados mostram a necessidade de se realizar algumas mudanças nas grandezas numéricas para que um perfeito diagnóstico e planejamento possam ser realizados em brasileiros, criando assim o padrão do perfil facial do brasileiro leucoderma de descendência européia.


AIM: Considering that modern orthognathic surgery is mainly concerned in planning and diagnosing clinical cases by the use of patients' soft tissue measurements obtained from digital images used in planning software, the aim of this study is to establish Caucasian Brazilians measures and to compare them to the standard North American measures. METHODS: For this, a total of 31 Angle's Class I patients not previously submitted to orthodontic treatment and presenting harmonious facial features were invited to participate in the study as volunteers. The patients were initially photographed in frontal and lateral views and, afterwards, cephalograms were obtained. The images were digitized, adjusted and indexed in Dolphin's image software. In order to precede cephalometric analysis, a total of 16 soft tissue profile points and 22 hard tissue profile points were demarcated, accordingly to American original measurements used in Dolphin's digital image program. After, all cephalograms were again evaluated to determine intra-examiner error according to paired Student's t test. Mean and standard error measures were obtained and compared to American measures by unpaired Student's t test with 95 percent of confidence level. RESULTS: The results obtained have shown that all measurements, except for 4 points in male and 4 points in female, are significantly different between Brazilian and American samples. Data show that Brazilians' soft tissue profile is almost completely different from American's, with a less protrusive face, shorter chin projection and a more convex profile. CONCLUSION: This suggests the necessity of performing some changes in numerical values to obtain an ideal diagnosis and planning of orthognathic surgery in Brazilians.

O objetivo deste trabalho foi avaliar o efeito do PRP no reparo ósseo, em combinação com osso autógeno ou não, em cães. Quatro cavidades ósseas no corpo da mandíbula foram preenchidas com coágulo, PRP, osso autógeno ou osso autógeno particulado associado ao PRP. Comesta metodologia foi possível concluir que o PRP não mostrou efeito positivo no reparo das cavidades ósseas quando usado sozinho ou em combinação com o osso autógeno

The objective of the present study was to determine the reproducibility of the pixel values obtained with the Digora system (Soredex, Finland). Exposures were standardized, with variation in exposure and scanning time of two photostimulable phosphor plates containing a stepwedge image. The smallest variation in pixel values ranged from 50 to 75 percent, with the widest variations being observed in less dense steps. A significant difference in pixel values was observed in terms of X-ray exposure and scanning times and between the two plates themselves (ANOVA, p < 0.01). Using the present methodology, the reproducibility of pixel values was not satisfactory for the tested white photostimulable plates. This wide variation in digitalization might be influenced by the amount of X-rays that sensitized the plates. It may be important to establish the reproducibility of the pixel values in quantitative studies using digital image.


O objetivo do presente trabalho foi determinar a reprodutibilidade dos valores de "pixels" obtidos com o sistema Digora (Soredex, Finlândia). As exposições foram padronizadas, com variação no tempo de exposição e leitura de duas placas de fósforo fotoestimuláveis contendo a imagem de um penetrômetro. A menor variação nos valores de "pixels" foi de 50 a 75 por cento, sendo as maiores variações oriundas nas faixas mais claras. Uma diferença significante nos valores de "pixels" foi observada em relação ao tempo de exposição e leitura das placas e entre as duas placas (ANOVA, p < 0,01). Utilizando a presente metodologia, a reprodutibilidade dos valores de "pixels" das placas testadas do sistema Digora não foi satisfatória. Essa grande variação na digitalização pode ter sido influenciada pela quantidade de Raios X que sensibilizou as placas. Isso pode ser importante para estabelecer a reprodutibilidade dos valores de "pixels" em estudos quantitativos usando imagem digital.
